Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements. You can also click here to view other options our office provides.

Dental Insurance

For patients with verified dental insurance – you will be responsible for the uninsured (or out-of-pocket) portion of any charge. At the time of treatment, you will be required to satisfy our estimate of the amount that your insurance company will not cover.  We will gladly bill your insurance carrier for you, and you will remain responsible for any portion not covered by the insurance carrier and not covered by any earlier payment you made.
